Key Factors in Implant Treatment Planning
According to Dr. Devi, the most important aspects in dental implant treatment planning are:
- Evaluation of the patient's medical history to assess risk factors
- Presence of adequate foundation bone
- How many teeth are missing and whether soft tissue is missing as well
- Whether the patient needs facial support with implant restoration
- What kind of restoration is the most appropriate, conservative, and affordable for the situation presented
The Implant Treatment Process
Implant treatment is typically divided into three steps.
Step I
Extraction and Bone Graft
When a patient presents with teeth that are severely decayed, broken, damaged, or infected, they need to be extracted. Sometimes the infection has spread to many teeth and the underlying bone is depleted. A CBCT scan is taken to evaluate the condition of the tooth and supporting bone to make the correct treatment decisions.
The bone graft procedure is done typically at the time of tooth extraction to fill the extraction socket, repair damage to the jaw bone, and create a solid foundation for future implants. The bone graft material is a mixture of allograft-xenograft mixed with growth factors from the patient's own blood. The typical healing time is 4–5 months. Patients may wear a retainer to cover the extraction site so they can continue with their normal routine.
Step II
Implant Placement
The extraction and bone graft site is ready for implant placement approximately 4–5 months after the initial surgery. A CBCT scan of the jaw bone will be performed to evaluate the healing of the extraction site and the quantity and density of the jaw bone at the site.
A digital guide will be made for precision implant placement considering the available bone and emergence profile of the new implant tooth. The usage of the digital guide avoids many potential problems with implant placement and the future restoration. The implant placement surgery is a minor surgical procedure. The implant is screwed into the jaw bone at the prescribed location and pre-determined angle, and the patient is allowed to heal for 3 months.
Step III
Restoration of the Implant
Restoration is the third and final step of the dental implant treatment process. The best restoration option will depend on factors like how many teeth are missing, the patient's need for facial support, and what is considered the most appropriate, conservative, and affordable for the situation presented.
Restoration Options
Single Implants
A single implant crown replaces one missing tooth with a natural-looking, permanent restoration that functions just like a real tooth.
Overdenture with Two Locators
Two implants with locator attachments provide a stable, removable overdenture — a significant upgrade from traditional dentures.
Overdenture with Four Locators
Four implants with locator attachments offer even greater stability and retention for patients who need more support.
Bar Overdenture
A custom bar connects multiple implants, providing maximum stability for a removable overdenture with superior fit and function.
All-on-4 / Teeth in a Day
Four strategically placed implants support a full arch of teeth — a life-changing solution for patients missing most or all of their teeth.
Prettau Bridge
A full-arch, fixed zirconia bridge supported by implants — the most durable and esthetic full-mouth restoration available.
